We are currently recruiting a Care and Support Assistant on a 35 hour contract, to join our expanding Targeted Care and Enablement Service. As a relief Care and Support Assistant, you will have the freedom and flexibility to work on a basis that suits you.
About the Targeted Care and Enablement Service:
Your role will provide a short intensive enablement service for people with disabilities and those who are frail and recovering from illness or injury, focusing on helping people to restore optimum level of independent functioning. The role will also provide a bridging service to those whose needs require ongoing home based care either at the end of their re-ablement period pending handover to an ongoing care provider, or to meet the long term care needs of people in urgent, challenging or fast response situations pending handover to an ongoing care provider.
The Service provides support for people who are experiencing a change in their ability to look after themselves following an accident or due to illness. The service offers a comprehensive range of personal and practical enablement care services for adults within Ceredigion, to enable them to continue living in the community, in accordance with their own lifestyle, and with independence, dignity, safety and respect. The key objective is to enhance the quality of life and relieve the stresses experienced by those who have reduced independence in their own home as a result of a disability, illness or age and offering an enabling service to maximise independence.
