An exciting opportunity has arisen for a number of Integrated Health and Social Care Support Workers to be part of an integrated fast acting community team (FACT) within Porth Gofal. ‘Porth Gofal’ supports an integrated holistic approach where services are person-centred and coordinated to the needs and preferences of the individual, focused on prevention and self-management, and meeting the needs of the local population in a timely, effective and prudent manner.
You will be working closely with the Acute Response Team, Therapy team, Enablement service, Local Authority, Social Services, Secondary Care and Third Sector colleagues. This is a fantastic time to join a dynamic innovative and rapidly transforming integrated service set in beautiful West Wales. The needs of the population are changing and these roles are integral to the delivery of care closer to home in line with the Discharge to recover and assess initiative. This dynamic and progressive service has been developed in recognition of these changing needs.
As an Integrated Health and Social Care Support Worker you will be working as part of the multi-disciplinary response team:
You will be expected to deliver high quality, evidence based Health and Social Care interventions with individuals in the community who require support to maintain their independence; responding to crisis, preventing admissions and supporting earlier discharge from the acute setting. The role will focus on supporting patient focussed pathways across community health and social care sectors.
The post holders will have the support of an integrated community team (Porth Gofal) which values supervision, actively promotes continuous professional development, and is committed to excellent patient care and service improvement.
We are therefore seeking to appoint proactive, motivated, and compassionate individuals, with demonstrable experience in community health or social care and whose values are aligned to those of Hywel Dda University Health Board and Ceredigion County Council:
