We are looking for a motivated and experienced Team Leader to join our Targeted Care and Enablement Service – a short-term, responsive service that plays a critical role in supporting people to regain skills, remain independent at home, and avoid unnecessary hospital admissions.
This is a dynamic and rewarding leadership role where you will lead a patch-based team delivering enablement, bridging, and fast-response care services across Ceredigion.
We are seeking someone with strong partnership, communication, and analytical skills, and experience in social care commissioning or service co-ordination. In return, you will join a supportive, forward-thinking council that values innovation, flexibility and community impact.
